Goodman Fielder Lock-Out Tag-Out Audit (17 sites)

Objectives

Goodman Fielder issued a new OH & S Lock-out tag-out policy, which required procedures to be developed for safely isolating machinery. Goodman Fielder tasked TEG with a full review of their plant across all machinery. This included:

  • Preparing a full register of appropriate isolation provisions for all energy sources including electricity, compressed air, steam and gravity.
  • Working with each site to develop LOTO standard operating procedures that included photos of the isolation points to enable staff to comply with the policy.

It was crucial that the project was delivered within very short turnaround time. TEG conducted the audit across 17 sites with over 1000 individual pieces of equipment being assessed. Each machine had multiple energy sources so in all several thousand assessment points were reviewed.The TEG Risk team also prepared capital expenditure requests to enable Goodman Fielder to obtain funds for upgrading machinery to the required standard to ensure compliance with the policy.  

A highly efficient project management approach was employed whereby templates were developed and piloted and TEG project engineers were rigorously trained to ensure the objectives could be met.

Results

  • TEG managed a wider team of experts to complete the work within a five week period.
  • Standard templates were developed for Goodman Fielder to ensure compliance with the policy and consistency of the data collected.
  • TEG also conducted Exception Risk Assessments with the sites to train them on this process.
  • Photos of all isolation points were taken and referenced on the register to allow the sites to develop the Standard Operating Procedures (SOP’s).
  • Detailed pricing schedules for plant upgrades were developed across all sites.
